NACDD supported recipients of CDC’s National Asthma Control Program (NACP) through the National Asthma Control Program Collaborative. Launched in 2023, this collaborative brought NACP recipients together to share promising practices, explore health information approaches, and identify creative ways to strengthen EXHALE strategies.
The support provided to NACP recipients focused on:
Providing opportunities for recipients and national partners to share expertise and improve asthma programs and services.
Exploring how electronic health records could support disease tracking and management.
Through facilitated discussions, NACP recipients explored approaches to monitor asthma burden within the EXHALE strategies.
Stories from the Field

The “Asthma Assistance to Health Departments, Including Electronic Health Record Use” project is supported by cooperative agreement number 6-NU38OT000286-05-03 through the Centers for Disease Control and Prevention of the U.S. Department of Health and Human Services (HHS) as part of a financial assistance award totaling $800,000 with 100 percent funded by CDC/HHS.
